| Creating an Opt-In Form for your Website |
Click > Clients > Forms
Click > Add Form
Click > Next
Step 2 shown below enables you to set what type of fields you want to use
(Text, Checkbox, List Box or Text Area)
Select the correct database field for each column, then click Import
Weight: This is how you order the fields on your form
List Box Data: If you choose Field Type List Box, this is where you would type in the selectable data that can be chosen for this field.
*You need to Click Save Form Attributes before selecting the next database field to edit
